Mental Health Matters Wales Secures Funding to Deliver Dementia Advocacy Service in Cardiff and the Vale of Glamorgan.

 

Mental Health Matters Wales (MHM Wales) is delighted to announce that we have been awarded funding to deliver a new Pilot Dementia Advocacy Service across Cardiff and the Vale of Glamorgan from the 1st  January 2025 to the 31st March 2025 . This vital service will provide targeted support for individuals living with dementia and their carers, ensuring they receive the guidance, care, and connections they need to live well throughout their dementia journey.

 

The Dementia Advocacy Service will act as a single, trusted point of contact for individuals and carers from diagnosis through to end of life, helping to navigate health, social care, and community support systems. Rooted in person-centred care, our service will promote independence, reduce social isolation, and ensure that individuals and families are empowered with knowledge, support, and access to local resources.

 

Delivered in close partnership with Cardiff and Vale University Health Board , Cardiff and the Vale of Glamorgan local authorities, and voluntary groups and third-sector organisations, this service aligns with the Welsh Government's commitment to enhancing dementia care through the All-Wales Dementia Pathway of Standards.

At Mental Health Matters Wales (MHM Wales), we are dedicated to supporting individuals living with dementia and their carers. Our Dementia Advocacy Service is designed to provide practical assistance, expert guidance, and a trusted point of contact throughout the dementia journey.

 

Our approach centres on promoting dignity, independence, and informed decision-making, ensuring that everyone affected by dementia can access the resources and support they need to live well.


Our Service:

 

The Dementia Advocacy Service offers a comprehensive range of support, including:


  • Tailored Guidance: Personalised support through "What Matters" conversations to identify and address individual and carer needs.
  • Service Navigation: Assistance in accessing health, social care, and community resources tailored to each person’s situation.
  • Future Planning: Help with advance care decisions, including Lasting Power of Attorney and planning for future care requirements.
  • Carer Support: Practical advice, respite connections, and peer support networks to alleviate the challenges faced by carers.
  • Community Engagement: Information on local dementia-friendly events, memory cafes, and meaningful activities to stay active and connected.
  • Crisis Response: Signposting and immediate guidance during unforeseen challenges, providing reassurance when it’s needed most.
